To lodge a claim, you must provide the following documents:

Type of claim	Supporting documents
Less than $12,000 or your claim is not genuinely contestable	Enter a summary of your claim into the form (less than 100 words), or Complete Form 1S Statement of Claim uploaded with Claim.
More than $12,000	Form 1S Statement of Claim uploaded with Claim.
Motor vehicle damage in the Magistrates Court	If the claim is for the cost of repair to your motor vehicle, and – the repairs are yet to be completed - a detailed quote from a repairer showing the cost of the repair of the motor vehicle; or the repairs have been completed – a detailed invoice of the costs of the repair of the motor vehicle. If the claim is for the loss of your motor vehicle – evidence of the value of the motor vehicle, being at least a valuation by a valuer or loss adjuster, or the RedBook value of the motor vehicle. If you are claiming for the cost of hiring a motor vehicle while your motor vehicle was being repaired or replaced – an invoice showing a detailed breakdown of teh cost of hire. If you are claiming any other costs you have had to pay as a result of the loss of your motor vehicle – detailed invoices showing a breakdown of these costs.
Property damage in the Magistrates Court	If the claim is for the cost of repair to your property, and – the repairs are yet to be completed - a detailed quote from a repairer showing the cost of the repair of the property; or the repairs have been completed – a detailed invoice of the costs of the property. If the claim is for the loss of your property – evidence of the value of the property, being at least a valuation by a valuer or loss adjuster. If you are claiming for the cost of hiring property while your property was being repaired or replaced – an invoice showing a detailed breakdown of the cost of hire. If you are claiming any other costs you have had to pay as a result of the loss of your property – detailed invoices showing a breakdown of these costs.
Building Work Contractors Act	Form 1AS Statement of Claim uploaded with Claim – Building Work Contractors Act (you choose to file a longer form statement of claim).
Retail and Commercial Leases Act	Form 1BS Statement of Claim uploaded with Claim – Retail and Commercial Leases Act (if claiming more than $12,000).
Second-hand Vehicle Dealers Act	Form 1CS Statement of Claim uploaded with Claim – Second Hand Vehicle Dealers Act (you choose to file a longer form statement of claim). A copy of the Agreement recorded by the Commissioner for Consumer Affairs (if one has been made). You may choose to provide the following: List of defects Copy of repair quote
Victims of Crime – Recovery	One of the following: Certificate under s28(3) Certificate under s28(5) Request for Summary Judgment Notice under s28(4) Notice of Request for Summary Judgment Draft Minutes of Order
